What types of insurance do you accept?
We accept Medicare, Triwest, Tricare Alliance, and most PPO insurances and Workers’ Compensation carriers. To see if we accept your healthcare insurance, please contact our office.
What to expect on your first visit
During the initial evaluation, a licensed physical therapist will gather a thorough history of your condition, perform various tests and measurements, and develop a plan of care tailored to meet your rehab goals.
Do I need a doctor’s referral?
Some healthcare insurances require a physician’s referral to receive physical therapy treatments. However, if you are paying cash for your physical therapy treatments, you are eligible to receive 12 visits of physical therapy or 45 days of therapy whichever occurs first. If therapy is warranted after that time period, patients must obtain a new referral from a Physician to continue physical therapy treatments.
**Note: Patients who have Medicare insurance must obtain a referral prior to a physical therapy evaluation.
Is there new patient paperwork to fill out?
Please arrive 15 minutes early to your first appointment to fill out pertinent new patient forms. If you would like to complete our patient forms prior to your first visit, click the link to print out your forms.
